That looks like an awesome tattoo idea/design. Very elegant and pretty.
I haven't had any done myself, but friends who have had them done in the past have told me that tattoo artists are very accommodating as they know it can be scary for a lot of people. Letting them know how you feel might also give them a better idea of how they can help you. Maybe even consider bringing a friend with you to keep you company during the whole process.
Also, consider using anesthetics to help with the pain, and don't feel ashamed for it (I know there is sometimes a weird stigma behind it - even though most people actually disagree with that stigma and support the use of anesthetics, especially for first-timers).
