I am sorry to hear you are feeling burnout and stress. I remember that feeling all too well when I was at school and so do my friends. Often we would try and take a day for ourselves when we could, that didn't have anything school-related whether it was once a month or when we went back to school every couple of months (mainly during the holidays). This way we were able to refresh ourselves and take care of ourselves. That could possibly help.
Try not to think about the high stakes and pressure if you can, often enough now there are multiple pathways to get what you want even if you don't achieve it the way you want to the first time, especially going into year 12. Try and enjoy year 12 (yes I know that's difficult) and take in the memories, the pressure and high stakes can seem less when you are enjoying yourself with it and importantly balance out your workload where you can. This can take away the pressure of it all.
Year 12 is hard especially when you are burnt out but taking care of yourself and enjoying it all is important.
